Eyebrow Transplant FAQ

EYEBROW · 29 QUESTIONS

Natural brows start with the right hair, placed the right way.

An eyebrow transplant is more than filling empty areas with darker hair. It means choosing hairs that complement your existing brows and placing them in the right position and natural direction for your face.

When I design eyebrows, I do not look at the brows in isolation. I consider the eyes and facial balance, the thickness and direction of the remaining eyebrow hairs, and the impression the patient wants to create. These questions and answers cover what is useful to know before deciding on surgery.

Start with what matters to you

A. Natural Appearance & Design

How is an eyebrow transplant different from an eyebrow tattoo?

Tattooing uses pigment to create the shape and color of an eyebrow. Transplantation moves hair follicles so that real hairs can grow. The main difference is the texture, direction and three-dimensional appearance created by actual hair.

An eyebrow transplant may be worth considering if a tattoo gives you the shape you want but still looks sparse up close, or if you want to add length and fullness to the tails. We start by identifying whether you want to change the color, the shape or the volume of actual hair.

Scalp hair is thicker than eyebrow hair. Can a transplant still look natural?

Yes. The key is not to take scalp hairs at random, but to select donor hairs that suit your existing eyebrows. Even on the same person, hair thickness varies from one area to another.

At Motential, we first measure the thickness of your existing eyebrow hairs with an AI hair microscope. We then identify suitable single-hair follicles and selectively harvest them one by one using FUE. We make active use of the ability to assess and choose individual follicles before harvesting them.

After reducing differences in thickness, we also match the direction and angle so the transplanted hairs blend with the existing brows. The goal is not automatically the finest possible hair or the highest possible density. It is a brow that suits your existing hair and has the definition you want.

What does the AI hair microscope do in an eyebrow transplant?

It magnifies the existing eyebrow hairs and potential donor hairs so we can compare their thickness. Rather than choosing hairs that simply look similar to the naked eye, we use measurements to help select suitable follicles.

I use those measurements to decide which hairs to place and where to place them. What matters more than the name of the device is how its findings are applied to the actual harvesting and transplantation.

What helps prevent transplanted hairs from sticking out or looking separate from the existing brows?

We need to match not just thickness, but also growth direction, the angle relative to the skin and the natural curve of the hair. Hairs at the inner brow, body and tail lie in different directions.

I place the hairs to follow the existing pattern in each area. To make the brows look natural even up close, we need to consider both the overall shape and how each individual hair lies. After surgery, we also assess the growing hairs and explain any grooming that is needed.

Can I bring a celebrity photo or a picture of eyebrows I like?

Yes. A reference image is useful for understanding the look you want. It can help us discuss whether you prefer softer brows, a more defined arch or a thicker, more masculine appearance.

What matters is how the design looks on your face, rather than copying someone else’s eyebrows exactly. We retain the features you like while adjusting the design to your eyes, facial proportions and the position of your existing brows.

Can you fill only the middle or tail of an eyebrow? Can asymmetry be corrected?

Yes. If you like the inner part of your brows but have a gap in the middle or short, faint tails, connecting just the areas that need attention can change the overall impression. Motential’s published cases include examples of improving the length and density of eyebrow tails.

When the two brows differ in shape, we can fill deficient areas to improve balance. We also check whether the difference comes from hair distribution, the way you open your eyes, or the position of the forehead muscles and underlying bone. The first step is to identify precisely what transplantation can change.

Are eyebrow designs different for men and women? Is surgery possible for older patients?

We do not choose a single shape based only on gender or age. Brow width, length, arch and tail direction can create different impressions on the very same face.

Some people want a strong, defined look, while others want soft, well-shaped brows without makeup. We design around the desired impression and the existing eyebrows. Surgical suitability is assessed by considering the skin, donor hair and general health together.

B. Tattoos, Scars & Suitability

Can I have an eyebrow transplant without removing my existing eyebrow tattoo?

Yes. An eyebrow tattoo does not automatically need to be removed first. If its shape matches the new eyebrow design or fits inside it, we can work with that design and add the texture and dimension of real hair.

If part of the tattoo extends beyond your preferred brow shape, we plan both the transplant area and the tattooed areas to retain. At Motential, our approach in these cases is to assess hair growth and the skin at least six months after transplantation, then consider selective laser removal of any pigment remaining outside the design if needed.

We review the removal area and laser approach together to protect the transplanted hairs.

Will a transplant remove an eyebrow tattoo that has turned red or blue?

Growing hair can add texture and volume to eyebrows that previously looked flat. However, an eyebrow transplant itself does not remove pigment from the skin.

We assess whether the discolored area falls within the new design and how visible it may remain between the hairs. We then plan separately for the changes transplantation can provide and any treatment the pigment may need. If tattoo removal is necessary, its timing and extent are decided after the postoperative assessment described above.

Can hair be transplanted into scars or areas that no longer grow after years of plucking?

We can assess whether transplantation can reconnect the empty areas. When a scar interrupts the eyebrow, the goal is to create a natural connection that matches the thickness and direction of the surrounding hairs.

Scar tissue can differ from normal skin, so we first assess its thickness, flexibility and healing. Rather than placing a large number of follicles immediately, it is important to plan density and any further reinforcement around the condition of that skin.

Can I have a transplant straight away if I have almost no eyebrows or they have recently been falling out?

Naturally sparse eyebrows and eyebrows that have recently begun to fall out need different assessments. For someone born with sparse brows, the main focus is designing the desired shape. If hair loss is ongoing, identifying its cause comes first.

When a condition such as alopecia areata or frontal fibrosing alopecia is involved, we assess whether treatment is needed first. Having almost no eyebrows does not by itself rule out surgery, but understanding why the hair was lost is essential to planning for the result to last.

C. The Procedure

Who designs the eyebrows, harvests the follicles and performs the transplant?

As the medical director, I personally provide the consultation, create the design, harvest the follicles and perform the transplantation. The thickness, hair pattern and desired impression agreed during the consultation need to carry through into the actual surgery.

The procedure is not simply about filling an outlined shape. I personally check whether the harvested hairs complement the existing brows and how each hair should be positioned in each area.

Where does the donor hair come from? Will you need to shave a large area, and will there be scars?

We select suitable follicles from donor areas such as the back of the scalp and harvest them individually using FUE. At Motential, we use a 0.7 mm fine punch to selectively harvest follicles for eyebrow transplantation. We focus on precisely obtaining the follicles needed while minimizing the impact on the donor area.

Our standard approach is partial shaving: only the area used for harvesting is trimmed short. Eyebrow transplantation requires relatively few follicles, so we plan for the surrounding hair to conceal the shaved area. We also adjust the harvesting locations and spacing so changes in fullness and harvesting marks are less noticeable. We do not remove a long strip of scalp, although small harvesting marks can occur.

A fully non-shaven procedure, without cutting the donor hair, is also available. The fees for partial shaving and a fully non-shaven procedure are explained separately in the pricing section below.

Do you use sedation? How much pain and swelling should I expect?

At Motential, eyebrow transplantation is performed under local anesthesia without sedation. There can be a brief sting when the anesthetic is administered, and we check for discomfort throughout the procedure. To limit swelling, we use only the minimum injection volume needed for surgery. In my clinical experience, most patients have very little postoperative swelling.

The postoperative pain level I generally describe to patients is mild, around 1 out of 10. If there is discomfort, it can be managed with Tylenol according to the instructions we provide. People experience pain differently, so please contact the clinic if it is stronger than expected or continues to increase.

How long does the surgery take?

Eyebrow transplant surgery takes approximately two hours. We work carefully through each stage, from choosing the follicles to placing them at the intended direction and angle.

When you book, we distinguish the surgery time from the total clinic visit, which also includes consultation and preparation. If you have other plans that day, please let us know before your visit.

How many hairs are usually needed?

The number depends on the hair already present and the area to be filled. Tail enhancement and full reconstruction of very sparse eyebrows require different numbers and distributions.

Motential’s published cases include a partial enhancement using 240 hairs and a case using 300 grafts. These numbers are not a standard prescription for every patient. The priority is placing the right hairs where they are needed, not simply increasing the count.

A hair count refers to individual hair strands; a graft count refers to transplanted follicular units. Although we select single-hair follicles for eyebrows, it is still important to distinguish the recorded units when reviewing a case.

D. Recovery & Grooming

When can I return to everyday activities? How much time off will I need?

Everyday activities are possible immediately after surgery. Being able to go about your day, however, is different from having no visible signs of surgery. Small crusts, redness or swelling may be visible at first.

For appearance-related recovery, it is best to allow at least a week. Recovery varies, so please tell us beforehand if you have an important photo session or face-to-face event. Exercise and sauna use are different from routine daily activities and can resume two weeks after surgery.

When can I wash or touch my eyebrows? What about exercise and saunas?

Gentle rinsing with running water is allowed. More important than water contact itself is avoiding pressure or rubbing over the transplanted area. Do not aim a strong stream of water directly at it or pick off crusts.

It helps to remember the care instructions in these stages:

First five days after surgery: do not touch the transplanted area with your hands.

After the first five days: very light contact is allowed if needed.

For the first two weeks: avoid direct pressure and rubbing. Being allowed light contact does not mean rubbing or massage is safe to resume.

Exercise and saunas: resume from two weeks after surgery.

Applying and removing eyebrow makeup is different from rinsing. We provide separate guidance after checking your recovery.

Does shedding of the transplanted eyebrow hairs mean the procedure has failed?

Some of the initially visible hairs may shed before new hair grows. Shedding of the visible hair shafts alone does not mean that the transplant has failed.

Rather than judging only by photos taken immediately after surgery and a few weeks later, we need to assess the subsequent growth. If you develop spreading redness, pain or pus, do not squeeze the area or pull hairs out. Contact the clinic promptly so we can assess whether you need an examination.

When will I see the result?

You can see the design and placement immediately after surgery. Over the following months, new hair growth changes the density and hair pattern. Assessing growth and overall balance over six to twelve months is an important part of the process.

A two-week photograph documents early healing and the design; it is not the final growth result. When viewing before-and-after photographs, always check how many months have passed since surgery.

Will I need to keep trimming the transplanted eyebrows? Will they last?

Yes. Because these are real growing hairs, they need to be trimmed to eyebrow length. Growth from established transplanted follicles is one of the key differences from creating a brow with pigment.

Transplanted scalp hair does not grow at the same rate in every person. You can adjust your grooming routine to your own growth rate and preferred length. We will advise when and how to trim for the first time based on your recovery. It is best understood as a procedure that creates brows with real hair, which you then maintain by keeping the length tidy.

Will I need to take hair-loss medication for life after an eyebrow transplant?

An eyebrow transplant does not, by itself, mean that everyone needs lifelong hair-loss medication. We assess treatment separately according to the cause of eyebrow loss and whether scalp hair loss is also present.

A short-term postoperative prescription and long-term treatment for a hair-loss condition serve different purposes. If you already take medication, we will review it with you during the consultation.

Can an unsatisfactory previous eyebrow transplant be corrected?

There are options for correction. The approach depends on whether the issue is insufficient density, hairs that are too thick or an unsuitable growth direction. In some cases, some of the existing transplanted hairs need to be removed before new hairs are added.

We plan revision surgery individually after examining the eyebrows and skin in person. Rather than simply adding more hair, I will explain what looks unnatural and which aspects can be changed.

E. Cost, Consultation & Visiting from Overseas

How much does an eyebrow transplant cost? Does the fee depend on the area treated or on shaving?

Motential’s standard eyebrow transplant surgical fee is KRW 2,000,000 with partial shaving of the donor area. The same fee applies to full eyebrow reconstruction and correction of selected areas.

If you choose a fully non-shaven procedure without cutting the donor hair, the fee is 30% higher than the standard surgical fee, at KRW 2,600,000.

Partial shaving: KRW 2,000,000 — the same fee for full reconstruction or partial correction.

Fully non-shaven: KRW 2,600,000 — 30% above the standard surgical fee.

With partial shaving, we trim only the necessary donor area and plan for the surrounding hair to conceal it. We can choose the approach together based on your current hair length, schedule and preferences. The follicle count guides the area and distribution of the transplant, rather than adding a per-hair charge.

How does the density touch-up policy work?

After an eyebrow transplant at Motential, the patient and I review the result at one year after surgery. If we jointly determine that further reinforcement is needed, the touch-up is performed at no additional charge.

We allow sufficient time for growth, then assess density and overall balance to identify the specific areas that need attention. I plan responsibly from the initial surgery through follow-up and any necessary reinforcement.

Can I have a photo consultation before visiting the clinic?

Yes. Helpful images include a front-facing photo showing both brows without makeup or filters, close-ups and angled views of each eyebrow, and a photo of the back of your scalp. If you have a reference image of the brows you would like, please send that too.

Please also tell us about any eyebrow tattooing or removal and any recent changes in eyebrow shedding. Photos help us establish an initial direction. The final design and surgical area are decided after examining the skin and hair in person.

How many days should overseas patients allow? Can follow-up continue after I return home?

An eyebrow transplant requires a two-day clinical schedule: the day of surgery and a follow-up check the next day. Before your visit, we review your desired design and current condition through photographs. Arrange your flights and accommodation after securing clinic appointments through the day after surgery.

After you return home, we can follow growth through photographs and online communication. We distinguish what can be assessed online from what requires an in-person examination and guide the next steps according to your progress. Please tell us your preferred surgery date and planned departure date before booking your flights.

Can I plan an eyebrow transplant alongside double-eyelid, nose or forehead surgery, or a hairline transplant?

The procedures can be planned together, but that does not mean every operation should take place on the same day or the following day. Surgery around the eyebrows can affect the design or transplanted area through swelling and the location of incisions.

If you have already booked another procedure, please tell us what it is and when it is scheduled. We will coordinate the order with the eyebrow position, skin condition and recovery in mind. We have published cases combining hairline and eyebrow transplantation, but whether to combine procedures and how to schedule them are individual surgical decisions.

Can I see before-and-after photographs of eyebrows similar to mine?

Yes. It is useful to look for cases that resemble your concern, such as generally sparse brows, tail enhancement or adding real-hair texture over an existing tattoo.

Rather than choosing the photo with the darkest brows, look at the starting point, the specific areas changed and the time elapsed since surgery. During your consultation, we can review published cases that are close to the change you want.

Let’s plan brows that belong on your face.

Eyebrows occupy a small part of the face, but the thickness and direction of each hair are easy to notice. At Motential, we measure the existing brows, select hairs that complement them and transplant those hairs in a pattern that suits the face.

If you are unsure which eyebrows would suit you, send us photographs of your current brows and the design you have in mind. We will start with a plan for what looks natural on your face, rather than simply a plan to transplant more hairs.
