“원장님, 작년부터 헬스를 본격적으로 했는데 그 뒤로 머리가 너무 많이 빠져요. 운동 때문일까요?”
“단백질 보충제 먹기 시작한 다음 달부터 빠지는 게 보여요. 프로틴이 탈모에 안 좋다는 게 사실인가요?”
“크레아틴 먹으면 DHT가 올라가서 탈모가 심해진다던데요. 그럼 끊어야 할까요?”
운동은 분명히 건강을 위한 좋은 습관입니다. 그런데 어느 순간 거울 앞에서 부쩍 늘어난 빠진 머리카락을 보면, 자연스럽게 가장 최근에 바뀐 습관 — 운동, 보충제, 식단 — 을 의심하게 되시죠. 그 마음 너무나 잘 압니다.
오늘은 운동과 탈모의 관계를, 인터넷에 떠도는 속설을 파헤쳐 의학적 팩트만 정리해 드리겠습니다.
1. 결론부터 말씀드립니다
운동 자체는 탈모를 유발하지 않습니다. 그리고 운동을 멈춘다고 해서 탈모가 호전되지도 않습니다.

다만 운동을 둘러싼 환경 — 갑작스러운 식단 변화, 수면 부족, 과도한 체중 감량, 일부 보충제, 그리고 가장 결정적으로 ‘내가 원래 안드로겐성 탈모 유전자를 가지고 있는지’ — 이 요인들이 결합되면 머리카락이 평소보다 더 많이 빠지는 것처럼 보일 수 있습니다.
그래서 정답은 “운동 = 탈모”가 아니라 “운동을 하는 시기에 다른 요인이 겹치면 탈모가 발생할 수 있다. “입니다. 이 차이를 이해하시면 다음 내용이 훨씬 명쾌하게 읽힙니다.
2. 가장 큰 오해 — ‘테스토스테론이 올라가서 머리가 빠진다’
운동·탈모 미신의 거의 모든 출발점은 이 한 문장입니다.
운동하면 남성호르몬 테스토스테론이 올라가고, 그게 DHT가 돼서 머리가 빠진다.
이 문장은 절반은 맞고 절반은 틀립니다.
먼저 맞는 부분. 남성형 탈모의 직접 원인은 디하이드로테스토스테론(DHT)이라는 호르몬이 모낭을 공격하는 것입니다. 그리고 DHT는 테스토스테론이 5알파-환원효소(5α-reductase)에 의해 변환된 형태입니다.

이제 틀린 부분. 운동으로 일시적으로 올라가는 혈중 테스토스테론은 보통 운동 직후 30분~1시간 내에 원래 수치로 돌아옵니다. 이 정도의 일시적 상승이 두피의 DHT 농도를 의미 있게 올린다는 근거는 사실상 없습니다. 그리고 더 중요한 점이 있습니다.
탈모의 핵심은 ‘혈중 테스토스테론 수치’가 아니라 ‘내 모낭이 DHT에 얼마나 민감하게 반응하도록 유전적으로 설정되어 있는가‘입니다.
같은 양의 DHT가 있어도 어떤 사람의 모낭은 평생 멀쩡하고, 어떤 사람의 모낭은 20대부터 가늘어집니다.
그 차이는 안드로겐 수용체(AR) 유전자에 의해 결정되며, 운동으로는 절대 바꿀 수 없습니다.
즉, 탈모 유전자가 없다면 아무리 헬스를 해도 그 이유로 머리가 빠지지는 않습니다.
3. 단백질 보충제(프로틴)는 정말 탈모를 일으키나?
결론부터 시원하게 말씀드립니다. 일반적인 양의 웨이(유청) 단백질이 탈모를 유발한다는 의학적 근거는 없습니다. 오히려 모발의 주성분이 단백질(케라틴)이기 때문에, 단백질이 부족할 때 모발이 가늘어지고 빠질 수 있습니다.

다만 두 가지 상황은 구분해서 봐야 합니다.
첫째, 일부 단백질 보충제에는 우유 호르몬, IGF-1 자극 성분이 들어있다는 주장이 SNS에 자주 등장합니다. 그러나 이는 동물성 우유 자체에 들어있는 미량 호르몬에 대한 과장된 해석이며, 사람의 두피에 의미 있는 영향을 준다는 임상적 증거는 부족합니다.
둘째, 보충제를 시작하면서 동시에 식단에서 탄수화물·지방·필수 미네랄(아연, 철분, 비오틴)을 급격히 줄이는 경우가 많습니다. 이때 발생하는 영양 불균형이 휴지기 탈모(Telogen Effluvium)를 일으키고, 환자분께서는 이걸 ‘프로틴 때문’이라고 해석하시게 됩니다. 진짜 원인은 보충제가 아니라 그 옆의 결핍입니다.
4. 크레아틴은 탈모와 관계있나요?
크레아틴은 운동·탈모 관련 보충제 중 가장 뜨거운 주제입니다. 2009년 남아프리카에서 진행된 한 연구에서, 크레아틴을 일정 기간 복용한 럭비 선수들의 혈중 DHT 농도가 평균 56% 정도 상승한 결과가 보고되었기 때문입니다. 대부분 이런 연구결과 하나가 수많은 기사를 양산하곤 합니다.

정확한 팩트는 이렇습니다.
먼저 이 연구는 표본 수가 적고(20명), 동일한 결과를 재현한 후속 연구가 충분하지 않습니다. 그리고 가장 중요하게도, 혈중 DHT가 올라간다고 해서 그 모든 사람이 실제로 탈모를 겪지는 않습니다. 다시 말씀드리지만, 탈모는 ‘DHT 수치’가 아니라 ‘모낭의 DHT 민감도‘에 의해 결정됩니다.
다만 안드로겐성 탈모 유전자를 명확히 가지고 있고 이미 진행 중이라면, 굳이 위험을 끌어올릴 필요는 없습니다. 이 경우 크레아틴을 줄이거나, 미녹시딜·피나스테리드 같은 표준 치료를 함께 가져가는 쪽이 합리적입니다. 탈모가 없거나 가족력이 전혀 없다면 일반 권장량(1일 3~5g)의 크레아틴은 일반적으로 안전하게 사용하실 수 있습니다.
5. 그렇다면 운동을 시작한 뒤 머리가 빠지는 진짜 이유는?
진료실에서 환자분의 사연을 자세히 들어보면, 운동
을 시작한 시점에 동시에 일어난 변화들이 진짜 범인인 경우가 대부분입니다. 임상에서 가장 자주 만나는 다섯 가지를 정리해 드립니다.

첫째, 급격한 체중 감량입니다. 한 달에 5kg 이상 빠지는 다이어트는 거의 예외 없이 휴지기 탈모를 부릅니다. 보통 다이어트 시작 2~3개월 뒤부터 머리가 빠지기 시작해서, 환자분께서는 운동 자체를 의심하시게 됩니다.
둘째, 단백질 외 영양소의 결핍입니다. 닭가슴살과 프로틴만 드시면서 철분, 아연, 비타민 D, 오메가-3가 빠지면 모발은 가장 먼저 신호를 보냅니다.
셋째, 수면 부족입니다. 새벽 운동 + 야근 패턴은 코르티솔(스트레스 호르몬)을 만성적으로 끌어올려 탈모를 가속화합니다.
넷째, 과훈련(Overtraining)입니다. 매일 고강도 + 충분한 회복 없음 = 만성 스트레스 상태이고, 이것 역시 휴지기 탈모의 흔한 원인입니다.
다섯째, 가장 결정적인 요인 — 단백동화 스테로이드(아나볼릭) 사용입니다. 일부 헬스장에서 음성적으로 권유받는 주사·경구 스테로이드는 명백히 탈모를 가속화합니다. 이 부분은 의사 입장에서 정말 강하게 말씀드립니다. 빠르게 강력하게 운동효과를 얻기 위해서 이 약을 사용함으로써 얻는 것은 일시적인 외형변화 뿐 장기적으로 잃는 것이 훨씬 큽니다.
6. 운동을 계속하면서 탈모를 관리하고 싶으시다면
운동을 멈출 필요는 전혀 없습니다. 다음 다섯 가지 원칙만 지키시면 됩니다.

첫째, 체중 감량은 한 달 2~3kg 이내로. 빠른 감량은 모낭이 가장 먼저 비명을 지릅니다.
둘째, 단백질만 보지 마시고 미량영양소(철분·아연·비타민 D)를 함께 챙기시기 바랍니다. 진료실에서 혈액검사로 한 번 점검하시면 가장 정확합니다.
셋째, 수면 7시간 이상은 그 어떤 보충제보다 효과적인 탈모 예방책입니다.
넷째, 본인이 탈모 가족력이 있거나 이미 빠지는 양상이 보인다면, 운동 시작과 함께 모발 검사를 받아 두시기를 권합니다. 운동 탓인지, 진짜 탈모가 시작된 것인지 4~6개월이 지난 뒤 알게 되면 그만큼 회복이 어렵습니다.
다섯째, 단백동화 스테로이드는 절대 금물입니다. 이건 타협의 영역이 아닙니다.
마치며 – 운동은 죄가 없습니다.
다만 운동을 시작한 시기와 탈모가 보이기 시작한 시기가 겹치면, 사람의 마음은 둘 사이를 자동으로 연결시킵니다. 그래서 의사로서 가장 자주 드리는 부탁은 이것입니다.

暖かくなり、夏に備える方々でジムが再び混み始めました。そしてこの時期になると、診察室に決まって登場する質問があります。
「院長、去年から本格的にジムに通い始めたのですが、その後、髪がすごく抜けるんです。運動のせいでしょうか?」
「プロテインを飲み始めた翌月から抜けるのが目立ちます。プロテインが薄毛に良くないというのは本当ですか?」
「クレアチンを飲むとDHTが上がって薄毛がひどくなると聞きました。やめるべきでしょうか?」
運動はまちがいなく健康のための良い習慣です。ところがある瞬間、鏡の前でぐっと増えた抜け毛を見ると、自然と最近変えた習慣 — 運動、サプリ、食事 — を疑ってしまいます。そのお気持ち、よく分かります。
今日は運動と薄毛の関係を、ネットに出回る俗説を掘り下げ、医学的な事実だけ整理してお伝えします。
1. 結論からお伝えします
運動そのものは薄毛を引き起こしません。そして運動をやめたからといって薄毛が良くなることもありません。
ただ、運動を取り巻く環境 — 急な食事の変化、睡眠不足、過度な減量、一部のサプリ、そして最も決定的に「自分がもともとアンドロゲン性脱毛の遺伝子を持っているか」 — これらの要因が重なると、髪が普段より多く抜けるように見えることがあります。
ですから正解は「運動=薄毛」ではなく、「運動をする時期に他の要因が重なると薄毛が起こり得る」です。この違いを理解すると、続きの内容がずっとすっきり読めます。
2. 最大の誤解 — 「テストステロンが上がって髪が抜ける」
運動・薄毛にまつわる迷信の、ほぼすべての出発点がこの一文です。

この文は半分正しく、半分間違っています。
まず正しい部分。男性型脱毛の直接の原因は、ジヒドロテストステロン(DHT)というホルモンが毛包を攻撃することです。そしてDHTは、テストステロンが5αリダクターゼ(5α-還元酵素)によって変換された形です。
次に間違っている部分。運動で一時的に上がる血中テストステロンは、通常、運動直後30分〜1時間以内に元の値に戻ります。この程度の一時的な上昇が、頭皮のDHT濃度を有意に上げるという根拠は事実上ありません。そしてもっと重要な点があります。
薄毛の核心は「血中テストステロンの値」ではなく、「自分の毛包がDHTにどれだけ敏感に反応するよう遺伝的に設定されているか」です。
同じ量のDHTがあっても、ある人の毛包は一生無事で、ある人の毛包は20代から細くなります。
その違いはアンドロゲン受容体(AR)遺伝子によって決まり、運動では決して変えられません。
つまり、薄毛の遺伝子がなければ、いくらジムに通ってもその理由で髪が抜けることはありません。
3. プロテイン(たんぱく質サプリ)は本当に薄毛を起こす?
結論からはっきりお伝えします。一般的な量のホエイ(乳清)たんぱく質が薄毛を引き起こすという医学的根拠はありません。むしろ髪の主成分はたんぱく質(ケラチン)なので、たんぱく質が不足すると髪が細くなって抜けることがあります。
ただし、二つの状況は分けて見る必要があります。
一つ目、一部のプロテインには牛乳ホルモンやIGF-1を刺激する成分が入っているという主張がSNSによく登場します。しかしこれは、動物性の牛乳そのものに含まれる微量ホルモンに対する誇張された解釈であり、人の頭皮に有意な影響を与えるという臨床的証拠は不足しています。
二つ目、サプリを始めると同時に、食事で炭水化物・脂質・必須ミネラル(亜鉛、鉄分、ビオチン)を急激に減らすことが多いです。このとき生じる栄養の偏りが休止期脱毛(Telogen Effluvium)を引き起こし、患者さまはこれを「プロテインのせい」と解釈してしまいます。本当の原因はサプリではなく、その隣の不足です。
4. クレアチンは薄毛と関係ありますか?
クレアチンは、運動・薄毛に関するサプリの中で最もホットな話題です。2009年に南アフリカで行われたある研究で、クレアチンを一定期間摂取したラグビー選手の血中DHT濃度が平均56%ほど上昇したという結果が報告されたためです。たいてい、こうした研究結果が一つあると、数多くの記事が量産されます。
正確な事実は次のとおりです。
まず、この研究は標本数が少なく(20名)、同じ結果を再現した後続研究が十分ではありません。そして最も重要なことに、血中DHTが上がったからといって、その全員が実際に薄毛になるわけではありません。繰り返しますが、薄毛は「DHTの値」ではなく「毛包のDHT感受性」によって決まります。
ただし、アンドロゲン性脱毛の遺伝子を明確に持っていて、すでに進行中であれば、あえてリスクを引き上げる必要はありません。この場合はクレアチンを減らすか、ミノキシジル・フィナステリドのような標準治療を併せて行うほうが合理的です。薄毛がない、または家族歴がまったくなければ、一般的な推奨量(1日3〜5g)のクレアチンは通常、安全に使用できます。
5. では、運動を始めた後に髪が抜ける本当の理由は?
診察室で患者さまのお話を詳しく伺うと、運動
を始めた時期に同時に起きた変化が本当の犯人であることがほとんどです。臨床で最もよく出会う五つを整理します。
一つ目、急激な減量です。1か月に5kg以上落ちるダイエットは、ほぼ例外なく休止期脱毛を招きます。たいていダイエット開始2〜3か月後から髪が抜け始めるため、患者さまは運動そのものを疑ってしまいます。
二つ目、たんぱく質以外の栄養素の不足です。鶏むね肉とプロテインばかり摂って、鉄分・亜鉛・ビタミンD・オメガ3が抜けると、髪が最初に信号を送ります。
三つ目、睡眠不足です。早朝の運動+残業のパターンは、コルチゾール(ストレスホルモン)を慢性的に押し上げ、薄毛を加速させます。
四つ目、オーバートレーニング(過剰な運動)です。毎日高強度+十分な回復なし=慢性的なストレス状態であり、これも休止期脱毛のよくある原因です。
五つ目、最も決定的な要因 — タンパク同化ステロイド(アナボリック)の使用です。一部のジムで非公式に勧められる注射・経口ステロイドは、明らかに薄毛を加速させます。この点は医師として本当に強くお伝えします。速く強烈に運動効果を得るためにこの薬を使って得られるのは一時的な外見の変化だけで、長期的に失うもののほうがはるかに大きいのです。
6. 運動を続けながら薄毛を管理したいなら
運動をやめる必要はまったくありません。次の五つの原則を守るだけです。
一つ目、減量は1か月2〜3kg以内に。急な減量は毛包が真っ先に悲鳴を上げます。
二つ目、たんぱく質だけでなく、微量栄養素(鉄分・亜鉛・ビタミンD)も一緒に摂ってください。診察室で血液検査を一度受けると最も正確です。
三つ目、睡眠7時間以上は、どんなサプリよりも効果的な薄毛予防策です。
四つ目、ご自身に薄毛の家族歴がある、またはすでに抜ける兆候が見えるなら、運動を始めると同時に毛髪検査を受けておくことをおすすめします。運動のせいか、本当の薄毛が始まったのかを4〜6か月たってから知ると、その分 回復が難しくなります。
五つ目、タンパク同化ステロイドは絶対に厳禁です。これは妥協の余地がありません。
おわりに — 運動に罪はありません。
ただ、運動を始めた時期と薄毛が見え始めた時期が重なると、人の心は自動的に二つを結びつけます。だから医師として最もよくお願いするのはこれです。
運動を疑う前に、正確な検査を一度だけ受けてください。アンドロゲン性脱毛なのか、休止期脱毛なのか、栄養不足なのか、甲状腺・ホルモンの問題なのかを見分けることが治療の始まりであり、その道を取り違えると、運動をやめても髪は抜け続けます。

Started working out and my hair is falling out? —
The real relationship between fitness, weights, and hair loss, explained accurately by a doctor
Hello. I am Dr. Injae Lee of Motential Clinic in Cheongdam, a physician who treats hair-loss patients every day and personally untangles mistaken folklore whenever it walks into the consultation room.
As the weather warms, gyms are getting crowded again with people preparing for summer. And around this time, there is a question that unfailingly appears in the clinic.
“Doctor, I started working out seriously last year, and since then my hair has been falling out so much. Could it be because of exercise?”
“I started seeing it fall out the month after I began taking protein supplements. Is it true protein is bad for hair loss?”
“I heard taking creatine raises DHT and worsens hair loss. Should I stop then?”
Exercise is clearly a good habit for health. But when at some point you see, in front of the mirror, a marked increase in fallen hair, you naturally suspect the most recently changed habit — exercise, supplements, diet. I understand that feeling very well.
Today I will dig into the folklore floating around online and lay out only the medical facts about the relationship between exercise and hair loss.
1. Let me give the conclusion first
Exercise itself does not cause hair loss. And stopping exercise does not improve hair loss either.
However, the environment surrounding exercise — a sudden diet change, lack of sleep, excessive weight loss, some supplements, and, most decisively, whether you originally carry the androgenetic hair-loss genes — when these factors combine, hair can appear to fall out more than usual.
So the answer is not “exercise = hair loss” but “if other factors overlap during the period you exercise, hair loss can occur.” If you understand this difference, the following reads much more clearly.
2. The biggest misunderstanding — “testosterone goes up and hair falls out”
Almost every starting point of exercise–hair-loss myths is this one sentence.

This sentence is half right and half wrong.
First, the right part. The direct cause of male-pattern hair loss is a hormone called dihydrotestosterone (DHT) attacking the follicles. And DHT is the form testosterone is converted into by 5-alpha reductase (5α-reductase).
Now the wrong part. The blood testosterone that rises temporarily from exercise usually returns to its original level within 30 minutes to an hour after exercise. There is essentially no evidence that this degree of temporary rise meaningfully raises the DHT concentration of the scalp. And there is a more important point.
The core of hair loss is not the “blood testosterone level” but “how sensitively my follicles are genetically set to respond to DHT.”
Even with the same amount of DHT, one person’s follicles stay fine for life, while another’s follicles thin from their 20s.
That difference is determined by the androgen receptor (AR) gene and can never be changed by exercise.
In other words, if you do not have the hair-loss genes, no matter how much you work out, your hair will not fall out for that reason.
3. Do protein supplements really cause hair loss?
Let me say the conclusion refreshingly. There is no medical evidence that a normal amount of whey protein causes hair loss. On the contrary, because the main component of hair is protein (keratin), when protein is lacking hair can thin and fall out.
That said, two situations must be distinguished.
First, the claim that some protein supplements contain milk hormones and IGF-1-stimulating components often appears on social media. But this is an exaggerated interpretation of the trace hormones present in animal milk itself, and there is insufficient clinical evidence that it meaningfully affects a person’s scalp.
Second, many people, when starting supplements, simultaneously and sharply cut carbohydrates, fats, and essential minerals (zinc, iron, biotin) from their diet. The nutritional imbalance that occurs at this time causes telogen effluvium, and the patient interprets this as “because of the protein.” The real cause is not the supplement but the deficiency next to it.
4. Is creatine related to hair loss?
Creatine is the hottest topic among exercise/hair-loss supplements. This is because a 2009 study in South Africa reported that the blood DHT concentration of rugby players who took creatine for a certain period rose by an average of about 56%. Usually one such study result spawns countless articles.
The accurate facts are these.
First, this study had a small sample size (20 people), and there are not enough follow-up studies reproducing the same result. And most importantly, a rise in blood DHT does not mean all those people actually experience hair loss. Again, hair loss is determined not by the “DHT level” but by the “follicle’s sensitivity to DHT.”
That said, if you clearly carry the androgenetic hair-loss genes and it is already progressing, there is no need to deliberately raise the risk. In this case it is reasonable to reduce creatine, or to carry standard treatment such as minoxidil and finasteride alongside it. If you have no hair loss or no family history at all, creatine at the general recommended amount (3–5 g a day) can generally be used safely.
5. So what is the real reason hair falls out after starting exercise?
When I listen carefully to a patient’s story in the clinic, the changes that happened at the same time
as starting exercise are usually the real culprit. Let me lay out the five I meet most often in practice.
First, rapid weight loss. Dieting that drops more than 5 kg a month almost without exception brings on telogen effluvium. Hair usually starts falling out 2–3 months after starting the diet, and the patient ends up suspecting the exercise itself.
Second, deficiency of nutrients other than protein. If you eat only chicken breast and protein while missing iron, zinc, vitamin D, and omega-3, the hair sends the first signal.
Third, lack of sleep. An early-morning-workout + overtime pattern chronically raises cortisol (the stress hormone) and accelerates hair loss.
Fourth, overtraining. High intensity every day + no sufficient recovery = a chronic stress state, and this too is a common cause of telogen effluvium.
Fifth, the most decisive factor — the use of anabolic steroids. The injectable and oral steroids covertly urged at some gyms clearly accelerate hair loss. On this part I speak very strongly as a doctor. What you gain by using these drugs to get exercise results quickly and powerfully is only a temporary change in appearance, while what you lose in the long term is far greater.
6. If you want to keep exercising while managing hair loss
There is absolutely no need to stop exercising. You only need to keep the following five principles.
First, keep weight loss within 2–3 kg a month. With rapid loss, the follicles scream first.
Second, do not look only at protein; take micronutrients (iron, zinc, vitamin D) together. Checking once with a blood test in the clinic is most accurate.
Third, 7 or more hours of sleep is a more effective hair-loss prevention than any supplement.
Fourth, if you have a family history of hair loss or already see a shedding pattern, I recommend getting a hair exam when you start exercising. Finding out 4–6 months later whether it was the exercise or real hair loss beginning makes recovery that much harder.
Fifth, anabolic steroids are absolutely forbidden. This is not an area for compromise.
In closing – exercise is not guilty.
However, when the time you start exercising overlaps with the time hair loss becomes visible, the mind automatically connects the two. So the request I make most often as a doctor is this.
Before you suspect exercise, please get an accurate test just once. Distinguishing whether it is androgenetic alopecia, telogen effluvium, nutritional deficiency, or a thyroid/hormone problem is the start of treatment, and if you take the wrong path, your hair will keep falling out even if you quit exercising.
